(A) Holders of WAEC, NECO or GCE O' Level with five (5) O' Level credit passes at not more than two sittings which must include English Language, Mathematics, Biology, Chemistry and Physics.
(B) Junior Community Health Extension Worker (JCHEW) with two (2) years post qualification experience in addition to the above mentioned requirement. Applicant must be registered with the Community Health Practitioners' Registration Board of Nigeria (CHPRBN) and posses a valid practice license.
